We do not collect information that personally identifies you unless you choose to provide it to us (for example, web pages that allow you to become a member of our practice, schedule appointments, request refills or contact us for any reason). 

The personal information you submit is shared only with those people in our organization who need this information to respond to your question or request, and as otherwise required to comply with applicable laws.

We also collect information that does not personally identify you, for various purposes (for example, web site usage, traffic patterns, visitor experience, site performance and related statistics based on your visits to our web site.)

Your Membership Log-In

It is your responsibility to safeguard your personalized account username and password against unauthorized use. Avicenna cannot be responsible for any such unauthorized use. However, should you find that such use could occur or has occur, please notify us so that we may provide new credentials and update our records.

Retention of Data

We will retain your information for as long as needed to provide you services and information you request or as permitted by applicable law.


We use reasonable security measures to protect the confidentiality of information under our control and appropriately limit access to it.  Avicenna cannot, however, ensure or warrant the security of any information you transmit to us, and you do so at your own risk.   Electronic transmissions via the internet are not necessarily secure from interception, and so we cannot absolutely guarantee the security or confidentiality of such transmissions.

Children’s Privacy

You must be at least 13 years old to have our permission to use this site.  We do not knowingly collect personal information from users under 13 years of age.  If you are under the age of 13, you can use this service only in conjunction with your parent’s or guardian’s permission.

Changes to This Policy

We may change or supplement our web privacy practices from time to time and we will post revised versions on this site.  Changes apply to the information collected from the date we post the revision, as well as to information we already hold.  We encourage you to frequently read this page so you are aware of any changes that may have been made since your last visit to the web site.

Governing Law

This policy is governed by the laws of the State of Pennsylvania, United States of America without regard to its conflicts of laws provisions.

Avicenna Direct Care is located in the State of Pennsylvania. The servers that support this site are located in the United States (US).  While it is in our possession, your information will  be stored in Avicenna databases or databases maintained by our third party service providers on servers and data storage devices located in the US.

Data maintained in the United States is subject only to privacy and security regulations in force within the US.  If you are located in a country outside the United States, by using this site you consent to the transfer of your information to the United States.
